How Do I Choose the Right DSO for My Dental Practice?

Choose a DSO based on total economics, cultural fit, clinical autonomy, staff treatment, integration approach, doctor compensation, equity structure and the buyer's ability to complete the transaction, not simply the highest headline valuation.

Start with your priorities

Before evaluating buyers, decide what matters most: maximum cash at closing, future equity upside, fewer administrative responsibilities, staff continuity, clinical autonomy, retirement timeline, continued leadership or a shorter employment requirement. Different DSOs fit different sellers.

Ask about clinical autonomy

Clarify who controls treatment planning, labs, materials, scheduling, clinical protocols, referrals and technology. Do not accept vague assurances when the issue is important to you.

Ask about your team

Are employees retained? Will benefits change? Who controls hiring? Are compensation changes expected? Is centralized HR involved immediately? Staff continuity often matters deeply to practice owners.

Understand integration

Some DSOs integrate quickly. Others preserve local branding and operations. Ask what happens to the practice name, phone systems, practice management software, billing, banking, website, marketing, vendors, payroll and scheduling.

Evaluate financial strength

A strong offer has little value if the buyer cannot close. Understand acquisition history, funding structure, lender requirements, timeline, approval process and outstanding contingencies.

Evaluate rollover carefully

A seller accepting equity becomes an investor. Evaluate the organization as an investment, not simply as an employer, and involve your own advisers.

Talk to affiliated dentists

Ask for doctors who sold practices to the organization two or three years ago. Did the DSO keep its promises? How did integration go? Did staffing improve or worsen? How much autonomy remained? Would they make the same decision again?

Identify buyers that align with both your practice characteristics and your personal priorities.
